Home + Community Re-Entry Program
When Nyna first returned home from hospital, she was dismayed to find herself unable to navigate the city because of her memory and problem solving problems. Step by step she worked diligently towards her goals of being independent using the public transit system, and today she is once again out enjoying luncheon engagements, visits to the library and other outings.
NRS offers a full range of assessments and therapies aimed at helping individuals like Nyna become as independent and active as possible in their own home and community, or in a transitional living setting or in nursing home. Many skills that are acquired at this stage are vital pre-requisites to quality of life and a successful return to work or school. Areas addressed include:
- Self care and daily routines (dressing, hygiene, feeding, mobility, sleep patterns, time management)
- Homemaking (cooking, housekeeping, laundry, budgeting)
- Community living skills (driving, TTC transportation, shopping, banking, etc.)
- Care giving (parenting, elder care)
- A-vocational pursuits (hobbies, social activities, volunteer work).
Programming may address difficulties with physical function, cognition, sensation and perception, communication, pychosocial skills and self -advocacy, as well as developmental/academic issues for children, adolescents and young adults.
The Home and Community Re-entry Team is comprised of:
- Case Managers/ Internal Co-ordinators
- Occupational Therapists
- Physiotherapists
- Speech Language Pathologists
- Social Workers
- Dietitian
- Rehabilitation Coaches
All staff have at least three years experience working with individuals with brain injury (or they are closely supervised by a professional with that experience).
