Case Management/Case Coordination
Following a traumatic brain injury, individuals may have any number of cognitive, communication, behavioural and physical deficits, which prevent them from carrying out their activities of daily life in the home, community, workplace or school. Thus, the rehabilitation process is often a long term one, even in the case of "milder" injuries. As a result, there is commonly a need for a qualified professional to act as a case coordinator to:
- Develop an initial Treatment Plan (we may assist the hospital in developing the plan; alternatively, it may be decided that it is more appropriate if we develop the initial plan once the client has been discharged and the community-based assessments are completed).
- Lead the team in developing appropriate long and short team goals and implementing a focused, integrated treatment program.
- Monitor the client's progress and coordinating the program over the long term.
We offer two types of case coordination services:
- Case management services (e.g. in CATASTROPHIC CASES).
- Internal Case Coordination Services by one member of the NRS treatment team (e.g. in NON-CATASTROPHIC CASES where a number of services need to be coordinated but funding is not available for a case manager).
NRS case managers/internal case coordinators provide rehabilitation counselling to the client and family to ensure their involvement and commitment to the program, act as a liaison for the insurer, and ensure effective communication between all members of the rehabilitation team. They ensure that concise, functional goal oriented reports are completed by each member of the treatment team, and that they are sent at regular intervals. NRS also offers "Goal Achievement Records" which are checklists designed to display, at a glance, the client's overall progress in all of his or her therapies over the previous reporting period.
Our case managers/internal case coordinators are ultimately accountable for the achievement of the long term rehabilitation goals, and maintaining a quality, cost effective rehabilitation program.
Neuro-Rehab Services' case managers/internal case coordinators are qualified professionals with a degree in either psychology, social work, occupational therapy, speech language pathology or physiotherapy, and have a minimum of three years brain injury rehabilitation experience. They also have at least two years brain injury case management experience (or are closely supervised by a professional with that experience).
